📋 Description
• Serve as the primary point of contact for patients after their intake visit. • Help patients understand and act on their care plan. • Resolve navigation friction. • Identify practical and social-needs support. • Coordinate with clinical team members involved in a patient's care. • Document every patient interaction accurately and on time.
🎯 Requirements
• Active, unrestricted RN license in good standing. • 3+ years of nursing or patient-facing experience. • Working knowledge of the U.S. healthcare system and Medicare. • Reliable computer, internet, and a quiet space for patient calls.
