• Complete in-home nursing assessments of eligible members • Conduct person-centered assessments focused on members' functional abilities and needs • Work with provider agencies to ensure services are delivered as authorized
• Valid driver’s license • Reliable home internet service • Current Montana Registered Nursing license • Experience with home health, hospice, or long-term care preferred
• Flexible schedule • Mileage reimbursement at the current federal rate • Full hourly wage for travel time
