📋 Description

• Monitor and clear daily patient lists • Route patients to assigned Care Navigators to ensure complete panel outreach • Track team progress toward check-in goals and monitor partner tracker spreadsheets • Review documentation logs for compliance with the mandatory minimum of 20 billable minutes • Manage the digital fax inbox • Process incoming medical records requests • Perform routine chart housekeeping within the EHR • Handle patient insurance eligibility and benefits verification • Send care plans, visit notes, and formal referrals • Request neuroimaging reviews upon receipt of patient images • Notify providers and Care Navigators when home safety assessments are completed • Send appointment confirmations and reminders • Research and locate local community resources under Care Navigator direction • Provide real-time administrative support to Care Navigators

🎯 Requirements

• 1–3 years of administrative or virtual assistant experience • Experience ideally within a care management, neurology, brain health, or busy medical clinic setting • Proficiency with electronic health records (EHR) • Proficiency with digital fax platforms • Proficiency with Excel or Google Sheets • Comfort working with numerical targets, outreach goals, tracking metrics, and minimum billable minutes • Exceptional written and verbal communication skills • Professional and empathetic communication with patients and partner organizations
