📋 Description

• Answer inbound calls, emails, and text messages from patients, caregivers, families, external providers, and facilities • Schedule and maintain contact with patients by phone or email • Explain health insurance benefits and eligibility; process copays and coinsurance • Follow up after appointments, provide next steps, and coordinate care • Ask screening questions and document patient safety, behavior, function, medications, care needs, advanced care plans, and caregiver issues under clinical supervision • Report screening results and escalate concerns to the clinical team • Implement care plans with patients, caregivers, healthcare providers, and community service providers • Send educational materials and information about Isaac Health and local community services • Provide emotional and psychological support to patients, caregivers, and families • Anticipate anxieties, provide supportive resources, and answer questions within scope of practice • Maintain participant privacy according to HIPAA regulations • Contribute to a supportive, mission-driven care team

🎯 Requirements

• Prior experience coordinating/navigating care for patients, caregivers, and their loved ones • Experience with patients who experience cognitive challenges is a bonus • Genuine empathy and patience • Exceptional time management and communication skills • More than 70% of the day spent on the phone • Ability to provide a personal computer/laptop meeting SOC2 security and telehealth standards • Ability to explain health insurance benefits and eligibility • Ability to process copays and coinsurance • Ability to work under clinical supervision and document patient and caregiver screening information • Ability to maintain participant privacy according to HIPAA regulations • Bilingual Spanish
