📋 Description

• Answer inbound calls, emails, and text messages from patients, caregivers, families, external providers, and facilities • Schedule and maintain contact with patients by phone or email • Explain health insurance benefits and eligibility; process copays and coinsurance as needed • Conduct and document patient and caregiver screening under clinical supervision • Report screening results to the clinical team • Implement care plans with clinicians, patients, caregivers, healthcare providers, and community service providers • Send educational materials and information about Isaac Health and local community services • Follow up after appointments and coordinate documented next steps • Request external medical records, laboratory results, and imaging results • Provide emotional and psychological support within scope of practice • Anticipate anxieties, provide supportive resources, and answer questions within scope of practice • Document patient care and instructions in the EHR • Coordinate care with external clinics, providers, and facilities • Escalate patient concerns to the clinical team • Maintain participant privacy according to HIPAA regulations

🎯 Requirements

• Prior experience coordinating/navigating care for patients, caregivers, and their loved ones • Genuine empathy and patience • Exceptional time management and communication skills • Spanish language skills are a bonus • Experience with patient populations who experience cognitive challenges is a bonus • Ability to explain health insurance benefits and eligibility • Ability to process copays and coinsurance • Ability to conduct screening questions and document patient safety, behavior, function, medications, care needs, advanced care plans, caregiver burden, depression, and coping under clinical supervision • Ability to document patient care and instruction in an EHR • Ability to coordinate care with external clinics, providers, and facilities • Ability to maintain participant privacy according to HIPAA regulations
