• Complete Initial Intake for new members to see how Nimble Health can best support their care journey • Review care history with patients with care navigation and/or education on Nimble Health services • Regular check-in calls to support the member’s care journey and track symptoms, treatment, and care satisfaction, and offer key Nimble Health services • Explain key and relevant program benefits to support the patient’s care journey • Offer care decision support, education material, navigation support and advocacy • Post surgery follow-up • Exercise clinical judgment in life and limb-threatening situations for proper connection and communication to the current treating provider or emergency medical services if warranted
• Must be Fluent in Spanish & English – This is a bilingual role • At least 2-3 years of relevant orthopedic or telephonic health coach experience • Experience in orthopedic care is preferred • Experience working in a telephonic role desired (call center, telephone triage, health coach or care manager) • Excellent interpersonal skills and ability to engage members • Hard-working and able to handle the pressure of a fast-paced team-based environment • Comfortable working with a remote team
