• Provide care coordination and clinical oversight for assigned patient caseloads across both agencies • Review, audit, and sign clinical documentation, including Starts of Care (SOC), Recertifications, Resumptions of Care (ROC), Discharges, and external physician orders • Serve as a point of contact for incoming clinical inquiries from field staff and patients • Offer guidance and escalate complex issues to the Directors of Patient Care Services (DPCS) as necessary • Work directly with physicians, healthcare providers, and community partners to maintain continuity of care • Analyze clinical documentation and workflows to identify quality improvement opportunities • Ensure alignment with agency policies and regulations • Support remote onboarding, training, and clinical guidance for new and existing field clinicians
• Active, unrestricted California Registered Nurse (RN) license • Minimum of 1-2 years of hands-on home health RN experience • Deep understanding of home health clinical workflows • Minimum of 1 year of supervisory experience in home healthcare or a similar field • Strong proficiency with Kinnser/WellSky EMR strongly preferred • Ability to fulfill the duties of this role remotely • Strong digital communication skills
