📋 Description

• Oversee and optimize OCHIN’s systems, processes, initiatives, and tools to support the success of members • Lead design and execution of program activities, ensuring timely delivery and strategic alignment • Build and maintain strong relationships with internal teams, leadership, and external partners • Support deployment of digital health tools to improve care access and outcomes • Apply advanced program and project management practices • Leverage data and analytics to drive decision-making and continuous improvement • Serve as the primary point of contact for program communications • Identify opportunities to improve adoption, implementation, and optimization of tools • Communicate regularly with HCOs to ensure program deliverables are meeting timelines

🎯 Requirements

• Bachelor's degree in program/project management or related field, master's preferred • Experience working with health care systems, patient portals, telehealth, e-consults, and/or remote patient monitoring technology required; experience with Epic functionality preferred • Knowledge of healthcare data, reporting, and analysis required • Familiarity with advanced primary care concepts, chronic disease management, clinical quality metrics preferred • At least 3 years' experience/knowledge of Interoperability landscape • Demonstrated ability to drive execution on multi-disciplinary projects • Strong communication skills with the ability to engage a wide range of roles • Strong analytical and problem-solving skills to support strategic decision-making • Strong experience in a telecommuter role required
