• Monitor and evaluate drug therapy for appropriateness based on patient health status, institutional criteria/guidelines, and cost-efficient measures • Manage pharmacy services in assigned areas to improve patient outcomes • Provide drug information to healthcare personnel and participate in patient care rounds with medical teams • Handle Primary Care refill requests and expedite the refill process while promoting patient safety • Complete E-Consults related to drug information, dose adjustments, order navigation, and similar topics • Provide pharmacokinetic consultation, evaluate laboratory data, assess drug selection and dosing, and communicate findings to healthcare practitioners • Evaluate patient responses to drug therapy • Document interventions and conduct outcome studies/evaluations of clinical activities • Assist with developing pathways, best practices, and guidelines • Educate staff, students, patients, healthcare providers, and other healthcare professionals • Submit work for publication once every two years • Mentor residents, students, and new staff • Provide patient care across transitions of care beyond the hospital visit • Support staffing needs during inclement weather, power outages, computer downtime, or other situations • Adapt care and communication to the specific patient population and patient needs
• Completion of a PGY-1 Pharmacy Practice Residency, or 3 years of related specialty experience in lieu of PGY-1 Residency • Required license to practice Pharmacy by the LA Board of Pharmacy • Doctor of Pharmacy (PharmD) preferred • Completion of a PGY-2 Specialty Pharmacy Residency preferred • Ambulatory experience preferred • Computer skills and dexterity for patient-information data entry and retrieval • Proficiency with Windows-style applications and keyboard • Effective verbal and written communication skills • Ability to present information clearly and professionally to varying levels of individuals throughout the patient care process • Strong leadership and teaching skills • Ability to meet physical demands, including frequently exerting 10–20 pounds of force and occasionally up to 100 pounds
