📋 Description

• Deliver nursing care to patients and families across the health illness continuum in diverse health care settings • Support the centralized Medication Refill Call Center within the Patient Advice and Care Team • Perform initial, ongoing, and functional health status assessments • Collect, review, and document subjective and objective patient data • Identify and communicate assessment findings to appropriate healthcare team members • Develop, initiate, coordinate, and document individualized care plans and nursing interventions • Provide direct patient care and facilitate patients and families through the healthcare system • Evaluate and document patient responses and revise plans of care • Delegate nursing tasks according to criteria and state statutes and review delegated work • Coordinate continuity of care and participate in development of peers and other employees • Coach employees and provide appropriate feedback • Participate in process improvement activities and support clinical quality, customer satisfaction, and financial performance • Act as a patient advocate and collaborate with providers • Manage personnel, supplies, and equipment to maintain a safe and efficient environment • Communicate instructions, perform minor scheduling, inspect work, and serve as a resource for associates

🎯 Requirements

• High School or GED (Required) • BLS - Basic Life Support - American Heart Association • LPN - Licensed Practical Nurse - National Council of State Boards of Nursing • Preferred 3–5 years of LPN experience • Prefer a minimum of 1 year of Primary Care experience • Strong communication and critical thinking skills • Comfortable working with technology in a fast-paced, collaborative environment • Ability to work from Ohio
