📋 Description

• Respond to incoming patient and family questions and escalate emergent triage calls to the clinical team • Provide timely, accurate, and helpful information to patients • Proactively contact patients for longitudinal care management • Coordinate and schedule appointments and referrals • Facilitate communication and collaboration among healthcare providers, patients, and families • Serve as a liaison between services and external providers • Monitor patients’ progress and response to treatment, identifying changes or complications • Communicate escalations or new health risks to clinical leadership • Maintain accurate communication logs and comply with documentation standards • Participate in quality improvement and performance improvement projects • Identify and implement strategies to enhance care quality • Adhere to HIPAA regulations and maintain patient confidentiality • Ensure compliance with clinical guidelines and standards of care

🎯 Requirements

• Some College or associate degree in clinical sciences, Nursing, or a related field • Minimum two (2) years of experience collecting referral information in the healthcare market • Medical Assistant certification preferred, but not required • Experience with electronic medical record software • Proficient use of the Microsoft Office suite • Ability to multitask, prioritize, and manage multiple projects simultaneously • Excellent written and verbal communication skills • Ability to work independently or in a team setting
