📋 Description

• Treat patients via telehealth visits, including telephonic triage calls, video visit appointments, and email follow-ups • Participate in continuous learning during weekly Clinical Rounds and through other modalities • Collaborate with virtual and in-office teammates via daily huddles • Apply specific clinical training to provide exceptional virtual patient care • Provide virtual clinical care in concert with patients’ primary care providers as part of a 24/7, 365-day team-based service

🎯 Requirements

• Completed an accredited FNP program with a national certification • In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an outpatient primary care setting seeing patients of all ages (0+), OR 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+) • Currently licensed in CO with ability to obtain additional state licenses • Ability to work weekday and weekend shifts, including every other Saturday and/or Sunday • Availability for current shifts ranging from 7am–6pm PST and 11am–10pm PST • Excellent clinical and communication skills • Clinical proficiency in evidence-based primary care • Ability to effectively use technology to deliver high quality care • Ability to communicate with and provide care to individuals of all backgrounds • Openness to feedback and reflection • Ability to navigate uncertain situations with patients and colleagues • Readiness to adapt personal and interpersonal behavior to meet patient needs • Authorization to work in the U.S.
