📋 Description

• Treat patients via telehealth visits, including telephonic triage calls, video visit appointments, and email follow-ups • Participate in continuous learning through weekly Clinical Rounds and other modalities • Collaborate with virtual and in-office teammates via daily huddles • Use specific clinical training to provide exceptional virtual patient care • Provide virtual clinical care to One Medical patients in concert with primary care providers • Deliver evidence-based care through a team-based approach 24 hours a day, 365 days a year

🎯 Requirements

• Completed an accredited FNP program with a national certification • In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an outpatient primary care setting seeing patients of all ages (0+), OR 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+) • Currently licensed in CO with ability to obtain additional state licenses • Ability to work weekday and weekend shifts, including every other Saturday and/or Sunday • Ability to work current shifts ranging from 7am–6pm PST and 11am–10pm PST • Excellent clinical and communication skills • Passion for human-centered primary care • Ability to communicate with and provide care to individuals of all backgrounds • Ability to effectively use technology to deliver high-quality care • Clinical proficiency in evidence-based primary care • Desire to be an integral part of a team dedicated to changing healthcare delivery • Openness to feedback and reflection • Ability to confidently navigate uncertain situations with patients and colleagues • Readiness to adapt personal and interpersonal behavior to meet patient needs • Authorization to work in the U.S. verified through E-Verify
