• Assist MTM Pharmacists in delivering safe, effective, and affordable medication therapy • Deliver targeted interventions to assess medication appropriateness, optimize therapy, and improve clinical outcomes • Conduct patient-centered adherence monitoring and identify solutions to adherence barriers • Assist with medication-cost management through formulary and therapeutic interchanges, pill-splitting opportunities, and duplicate-therapy assessments • Receive and respond to patient calls, take protocol-based action, and escalate calls when needed • Document patient discussions and schedule follow-up • Update patient demographic, allergy, and medication information in the MTM platform • Obtain updated healthcare-provider contact information • Provide internal and external customer service • Perform auxiliary departmental functions and meet manager-set productivity and performance expectations • Engage prescriber offices to obtain responses to pharmacist-generated clinical recommendations • Collaborate with pharmacists, pharmacy technicians, and pharmacy students to resolve medication therapy problems • Document and submit claims for pharmacist clinical interventions • Explain MTM program benefits to patients and schedule Comprehensive Medication Reviews • Provide at least ten hours of availability per work week between 8 AM and 7 PM CST, Monday through Friday
• Minimum 1 year of work experience • High School Diploma • Active pharmacy intern or pharmacy technician license in the state of residence, where required • Active enrollment in PharmD program (second year preferred) • At least 1 year of outpatient pharmacy experience preferred • Strong telephonic skills • Excellent listening and interpersonal skills • Healthcare professional-level communication skills • Professional demeanor • Exceptional interviewing skills • Conflict resolution skills • Strong work values • Self-management skills, including self-motivation, organization, and attention to detail • Understanding of basic compliance measures for CMS programs and HIPAA privacy and confidentiality requirements • Proficiency in MS Office applications • Strong data entry skills • Ability to multitask using multiple database applications • Ability to provide excellent customer service • Proven ability to work independently in a collaborative environment
