📋 Description

• Provide comprehensive care management and step-down care support for individuals transitioning from hospitals and emergency departments • Identify opportunities for Paragon programs to support youth and families and help avoid out-of-home placements • Develop individualized, goal-oriented service plans with clients and clinicians • Coordinate with hospitals, external providers, community resources, clients, and local agencies to ensure continuity of care • Conduct community prevention, outreach, education, and engagement activities • Maintain current knowledge of local resources, services, and programs • Support Medicaid/Medicare-covered individuals and families in understanding recommended treatments • Generate referrals from community members, healthcare providers, and other sources within identified counties • Develop outreach, education, and engagement plans with leadership • Build and maintain relationships with local organizations, healthcare professionals, community leaders, law enforcement, hospitals, public health departments, and cultural organizations • Facilitate skill building and coaching, providing encouragement, empathy, and practical guidance • Advocate for individuals experiencing mental health challenges or substance use disorders • Ensure outreach activities comply with applicable laws, regulations, and ethical standards • Participate in community meetings, collaboratives, outreach events, tabling, and wellness activities • Travel to clients’ homes and throughout the community

🎯 Requirements

• Bachelor’s degree in social work, psychology, counseling, or a related field is preferred • Prior experience in community engagement, referral generation, care management, and/or peer support is highly valued • Experience in family support, recovery, and mental wellness is preferred • Excellent communication and interpersonal skills • Strong networking abilities • Empathy, compassion, and a nonjudgmental attitude • Organizational and time-management skills • Collaborative problem-solving and decision-making abilities • Valid driver’s license and proof of automobile insurance required • Bilingual Spanish preferred, depending on location • Willingness to travel to homes and communities • Reliable transportation, telephone, and email access required • Requires living in or near the location of the posting
