📋 Description

• Support the launch of behavioral health and medical services in Arizona • Participate in commercial and Medicaid payer enrollment processes • Complete and maintain CAQH and credentialing documentation • Review and sign provider enrollment applications and related attestations • Participate in occasional meetings related to network expansion and market launch • Participate in 1–2 user research interviews about the Arizona provider and patient care landscape • Identify local pediatric key opinion leaders and facilitate introductions where appropriate • Provide potential future clinical services after patient services launch, based on mutual interest and business needs

🎯 Requirements

• Arizona-licensed Pediatrician • Active, unrestricted Arizona medical license • Board-certified Pediatrician • Active DEA registration • Ability to credential with commercial and government payers • Arizona state residency required • Experience with telehealth and value-based care models preferred • Strong attention to detail and responsiveness during enrollment and credentialing activities • Active Arizona Medicaid enrollment preferred • Reliable, secure internet connection in a private, distraction-free setting for remote care • Pediatric care experience, including experience with ADHD/anxiety, obesity, and anti-obesity medications • Additional trainings, specialties, or certifications should be disclosed • Ability to start the 1099 Consultant role at an agreed time
