• Collect, validate, and clinically quality-review patient-level data for national clinical quality initiatives and clinical data registries • Manage the overall medical record retrieval process for abstracting required data • Collect complex data elements using established definitions • Ensure integrity of electronic data transmitted to national databases and registries • Prepare and interpret clinical data reports • Work with clinical team members on quality improvement projects • Translate clinical documentation into actionable information for improvement • Identify variances between and within systems and evaluate potential causes • Collaborate with leadership teams, quality leaders, subject matter experts, stakeholders, and ad hoc teams on data abstraction and performance improvement • Abstract clinical data from electronic medical records according to patient-selection criteria and sponsoring or regulatory entity guidelines • Develop efficient data collection and management processes • Validate data, clarify case questions, and maintain abstraction proficiency and productivity • Upload and submit data files; investigate and correct submission errors • Work with stakeholders to resolve software, report capacity, and benchmarking issues • Foster collaboration between abstraction staff and clinical leaders • Meet with clinical stakeholders to report quality outcomes, performance, and compliance results • Review medical records for deviations or non-compliance, identify trends and patterns, and provide stakeholder feedback • Perform other duties as assigned
• Associate degree; preferred education in a healthcare-related field, Health Information Management, or related field • In lieu of an Associate degree, five (5) or more years of directly related data abstraction experience may be considered • Two (2) years of clinical data abstraction, data management, or data entry experience preferred • Oncology Data Specialist Certification by the National Cancer Registrars Association (NCRA) required within one year of employment if working with the Cancer registry • Proficient computer skills, including word processing, spreadsheets, PowerPoint, databases, and data entry • Mathematical skills • Knowledge of electronic health records and electronic medical records • Working knowledge of quality improvement, clinical data review, and performance improvement methodologies • Ability to effectively communicate opportunities for improvement • Detail oriented • Ability to work independently and adhere to sponsoring agency/entity standards and definitions • Communication skills and ability to interact with and build relationships within quality improvement teams • Ability to use appropriate resources to make decisions regarding data abstraction, interpretation, and dissemination • Knowledge of human anatomy, medical terminology, data abstraction, and data entry
