📋 Description

• Provide care transition services for Providence hospitalized patients • Work with the Providence physician team and hospital discharge planning staff • Ensure patients are successfully discharged from the hospital without disruption in services • Perform nursing assessments • Serve as a liaison among patients, families, physicians, and care teams • Provide communication, education, and team/family support

🎯 Requirements

• Graduate from an accredited school of Nursing • Washington Registered Nurse License upon hire • Bachelor's Degree in Nursing (preferred) • 1 year clinical experience (preferred) • Experience providing patient and staff education (preferred) • Experience in conflict resolution (preferred)
