📋 Description

• Communicate with patients, health plans, and medical groups • Work directly with patients and key personnel at each ministry, Care Management, insurance companies, and community partners as needed • Provide excellent customer service • Exercise initiative and judgment in a busy environment • Follow up and follow through with patients and stakeholders • Maintain thorough knowledge of services throughout the Providence continuum • Act as a resource to the community

🎯 Requirements

• Master's Degree Social Work • Upon hire: California Clinical Social Worker License or California Marriage and Family Therapist License • Five years previous experience working in a healthcare role is required • Practical experience in customer service • Background in managed care health insurance or care management preferred
