📋 Description

• Perform comprehensive assessments to establish, confirm, and stage dementia diagnoses • Create and implement evidence-based care plans addressing clinical, behavioral, safety, and caregiver-support goals • Stay current with advancements in dementia care and share expertise across the team • Identify opportunities to improve the care model and technical platform • Manage a panel of patients with dementia and their caregivers longitudinally • Evaluate the distinct needs of patient-caregiver dyads and center interventions around their priorities • Monitor cognitive changes and environmental stressors to prevent acute escalations, unnecessary emergency department visits, and hospitalizations • Collaborate with the interdisciplinary team, including Neurologists, Geriatricians, Care Navigators, Registered Nurses, Nurse Practitioners, Licensed Clinical Social Workers, and Pharmacists • Partner with teams on clinical quality, utilization, and patient/provider satisfaction benchmarks • Participate in workflows supporting reliable 24/7 care access • Report to the Medical Director and support hiring and training new providers as the organization expands • Work cross-functionally to improve processes and tools for people living with dementia and their caregivers

🎯 Requirements

• Board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) or Adult-Gerontological Nurse Practitioner (AGPCNP-BC) • Graduate of an accredited US Nursing School and an accredited Nurse Practitioner Program • Proven clinical background in geriatrics, palliative care, or memory care • Deep experience in dementia and related cognitive disorders • Demonstrated experience in advance care planning and serious illness communication with patients and caregivers • Experience working with interdisciplinary care teams • Prior experience in a high-growth startup or digital health environment • Excellent communication in a remote-first environment • Comfort using asynchronous tools such as Slack and Notion • Comfort delivering care via telehealth • Strong problem-solving skills and a flexible, creative approach • Active, unrestricted license in OR, WA, ID, and OH • Strong leadership and communication skills • Passion for building, launching, and iterating on a novel care model and technical platform
