📋 Description
• Coordinate services and care for NHTD Waiver participants • Develop, review, and update service plans based on participants’ needs • Communicate with participants, families, caregivers, and service providers to support continuity of care • Monitor service delivery, follow up on outstanding needs, and address coordination issues • Maintain accurate case records and documentation in accordance with program requirements • Manage a caseload independently while maintaining timely communication and follow-up
🎯 Requirements
• Hands-on NHTD Waiver experience is required • Previous Service Coordination experience is preferred • Knowledge of NHTD regulations, service plans, and care coordination • Strong communication, organizational, and case management skills • Ability to work independently and manage multiple cases • A reliable, professional, and compassionate approach • Russian language proficiency is a plus
