📋 Description

• Make outbound calls to members based on identified health interventions or quality initiatives • Build rapport with members by communicating personal experience as a member and peer • Provide health education and encourage positive behavior changes • Coach members to create and implement strategies that minimize risks associated with common health conditions • Encourage and assist members who encounter obstacles to seeking care • Provide peer support, guidance, advocacy, resources, encouragement, and listening support for individuals facing mental health, addiction, or other life challenges

🎯 Requirements

• High School Diploma or Equivalent Experience • Current SCAN Membership, required • Proficiency operating a computer and navigating multiple systems, including online timesheets and call center software • Substantial experience with MS Office Products and internet browsing • Excellent communication skills, including ability to articulate personal SCAN membership experience • Strong interpersonal, written, and verbal communication skills • Ability to work independently and as part of a team • Strong organizational skills • Ability to work in a changing environment • Valid driver’s license, automobile insurance, and reliable transportation • Background check required
