📋 Description

• Provide integrated clinical pharmacy support to patients and physicians through personalized relationships • Improve medication adherence and promote evidence-based care • Provide medication management, reconciliation, dose modification/optimization, laboratory test ordering, and adherence counseling under approved protocols/CPA/CDTM • Develop individualized care plans with interdisciplinary teams • Monitor medication regimens for therapeutic goals, adverse effects, drug interactions, and clinical criteria • Identify drug therapy optimization and medication cost-saving opportunities • Conduct in-person and virtual/phone disease-state assessments and drug therapy education • Participate in clinical outcome studies, REMS compliance/reporting, and process-improvement projects • Provide clinical justification and supporting literature for prior authorizations and appeals • Coordinate medication management with physicians, specialists, and onsite pharmacists • Support peer-to-peer requests and targeted medication assessments • Ensure compliance with FDA guidance, clinical guidelines, and evidence-based literature • Optimize workflows and collaborate on operational changes • Promote patient safety, medication literacy, education, and counseling • Maintain confidentiality of organizational and patient information • Participate in professional education, publications, organizations, meetings, conferences, and development activities • Perform other duties assigned by the Senior Director of Clinical Services and Manager of Clinical Services

🎯 Requirements

• Doctor of Pharmacy degree (PharmD) from an accredited school of pharmacy • Licensed pharmacist in Colorado • One of: certification in a relevant field of practice; completed an accredited postgraduate residency with at least 50% direct patient care experience with interdisciplinary teams; or at least one year of clinical patient services under a collaborative practice agreement/protocol, collaborative drug therapy management, hospital pharmacy practice, or health system • Complete continuing education requirements to maintain Colorado pharmacist licensure if selected • Ability to analyze information and exercise good judgment • Strong verbal communication skills • Deep knowledge of pharmacology developments and FDA health regulations • Strong technical and clinical skills • Legal compliance, productivity, quality, and attention to detail • Strong clinical knowledge and validated expertise in disease-state management and treatment guidelines • Organized, high-integrity, dependable, empathetic, and discreet • Solid computer skills, including Microsoft Outlook, Teams, Word, Excel, PowerPoint, internet, and web-based platforms • Data entry and computer skills evaluated during pre-employment assessment
