📋 Description

• Provide integrated support to patients and physicians • Develop personalized relationships with patients and prescribers • Review and interpret clinician's patient care notes for supportive clinical data • Develop disease and drug specific clinical protocols • Monitor ongoing patient adherence across all sites • Evaluate EMR to review interactions, medication selection, labs, allergies, and communication • Recommend alternative therapeutic drug regimens • Monitor therapies for side effects and recommend changes • Manage and document medication therapy management • Oversee tech's daily activities • Identify and resolve insurance related issues • Maintain HIPPA compliance • Train pharmacy staff, students, interns, and healthcare professionals • Assist with research projects and peer reviewing articles

🎯 Requirements

• Required Licensure/Certification: Applicant must hold a Doctor of Pharmacy (PharmD) or equivalent advanced degree and be a licensed Pharmacist • 1+ years' experience is strongly preferred but not required • Knowledge using EPIC/QS1/WILLOW software a plus
