📋 Description

• Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards • Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a Collaborative Practice Agreement (CPA) and promote seamless medication coordination • Monitor therapy effectiveness and safety, identify optimization opportunities, and intervene as appropriate • Support quality-improvement and REMS initiatives • Support teammates with complex prior authorizations, improve workflows, educate patients, and maintain ongoing clinical development • Build relationships with patients and providers through virtual patient assessments and evidence-based treatment-plan monitoring

🎯 Requirements

• Hold a Doctor of Pharmacy (PharmD) • Must be a licensed pharmacist in Washington • Completion of a PGY2 Ambulatory Care Residency, or completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA), or at least 3 years of ambulatory practice with disease state management • Strong communication, organization, and tech skills • Empathy, integrity, and a team-first mindset • Solid computer skills required, including proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint) • Ability to navigate the internet and web-based platforms • Data entry and computer skills will be evaluated through a pre-employment assessment
