• Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments aligned with evidence‑based standards. • Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination. • Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate. • Support quality‑improvement and REMS initiatives. • Provide recommendations to operations by supporting teammates as needed with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.
• Hold a Doctor of Pharmacy (PharmD) • Must be licensed pharmacist in California • Candidate must meet one of the following qualifications: • Completion of a PGY2 Oncology Residency • Completion of PGY1 Ambulatory Care Residency and at least 3 years of experience as a clinical oncology pharmacist • Have at least 3 years of experience as a clinical pharmacist in oncology, including 1 year practicing under a Collaborative Practice Agreement (CPA) • Strong communication, organization, and tech skills • Empathy, integrity, and a team-first mindset
