📋 Description

• Support Medicare members remotely throughout the full arc of their care • Use LPN/LVN clinical knowledge while learning and using Solace’s proprietary platform, tools, and standards • Build trusting patient relationships through attentive listening and empathy • Assess patient situations, prioritize needs, and create care plans • Address clinical and non-clinical barriers, including food access, transportation to appointments, and in-home support • Guide patients through care coordination, resource navigation, and administrative challenges • Connect with patients by video or phone without in-person interaction

🎯 Requirements

• Current LPN or LVN license in good standing • Applicants must be based in the United States • Track record of working with patients across diverse backgrounds and communities • Compassion rooted in patient care and advocacy • Ability to organize multiple priorities and remain steady in a fast-paced environment • Ability to learn unfamiliar technical systems quickly • Ownership and ability to work independently • Some weekly availability during core business hours Monday–Friday • Must be authorized to work in the United States • Prior care plan creation experience is not required
