📋 Description
• Conduct virtual wellness visits directly to patients in their homes • Engage in proactive care by conducting thorough health risk assessments and medication reviews • Collaborate with in-home team members to evaluate vital signs and arrange blood draws • Provide comprehensive care coordination and management, including preventive care interventions and referrals
🎯 Requirements
• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner License in GA, NC, SC, FL, MS • Strong written and verbal communication skills • Ability to work independently or in a team environment • Technologically savvy and comfortable using tools such as laptops for charting and HIPAA secure messaging apps • Pass national background check and valid clinical license search
