📋 Description
• Conduct virtual wellness visits to patients in their homes • Perform health assessments and cognitive screenings • Engage in proactive care including medication reviews and patient education • Collaborate with in-home team members for patient care coordination • Provide referrals to specialists and document findings
🎯 Requirements
• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner license in Louisiana • Strong written and verbal communication skills • Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination • Pass national background check and valid clinical license search
