• Conduct virtual wellness visits directly to patients in their homes • Engage in proactive care by conducting thorough health risk assessments • Provide comprehensive care coordination and management • Collaborate with clinical team members to evaluate health needs
🎯 Requirements
• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner License in Oklahoma • Strong written and verbal communication skills • Technologically savvy and comfortable using tools such as laptops or mobile devices for charting
