📋 Description

• Conduct virtual wellness visits directly to patients in their homes • Review case load and prepare technology for patient services • Navigate schedule and partner with in-home team members • Conduct health risk assessments, medication reviews, and cognitive screenings • Provide comprehensive care coordination and management • Engage with patients and create meaningful bonds

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner License in California • Credentialed Medicaid provider for the state of California (Medi-Cal) • Experience performing wellness visits • Problem-solving abilities • Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps • Strong written and verbal communication skills • Ability to work independently or in a team environment • Pass national background check and valid clinical license search
