📋 Description

• Review caseload and prepare technology for patient services • Navigate the daily schedule using technology • Partner with clinical in-home team members visiting patients’ homes • Build welcoming connections with diverse patients • Conduct virtual adult and/or pediatric wellness visits • Perform health risk assessments, medication reviews, and cognitive screenings • Provide patients with educational information about health and well-being • Evaluate vital signs and arrange blood draws with Sprinters • Provide care coordination and management • Deliver preventive care interventions, medication management, specialist referrals, community-resource referrals, and documentation

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner license in Louisiana • Prior experience performing wellness visits • Professionalism, integrity, accountability, confidentiality, care, and compassion • Willingness to perform last-minute problem solving and out-of-the-box thinking • Technologically savvy and comfortable using laptops, mobile devices, and HIPAA-secure messaging apps • Strong written and verbal communication skills • Ability to work independently or in a team environment • Pass national background check and valid clinical license search
