📋 Description

• Provide compassionate, patient-centered behavioral health support in a virtual setting • Provide comprehensive biopsychosocial and standardized assessments for potential program enrollment • Develop individualized support plans with patients and establish goals • Provide community service resources and make referrals • Provide enrollment and intake services • Respond to telephone inquiries about mental health care and services • Conduct safety assessments and planning by telephone or video • Partner with patients to create sustainable, self-directed changes that promote health and well-being • Collaborate with multidisciplinary team members to develop patient support goals • Participate in multidisciplinary patient care conferences as needed • Document interviews, consultations, referrals, contacts, and supportive services • Complete documentation and reporting according to departmental standards and federal and state regulations • Work in a constant state of alertness and safe manner • Perform other duties as assigned

🎯 Requirements

• Must live in the four-state footprint: Missouri, Illinois, Wisconsin, or Oklahoma • Master's degree in Behavioral Health or education equivalency for licensing • Required professional license/certification appropriate to the state of work location: Illinois — LCPC, LCSW, or LMFT; Missouri — LCSW, LMFT, or LPC; Oklahoma — LADC, LBP, LCSW, LADC-MH, LMFT, LMSW, or LPC; Wisconsin — LCSW, LMFT, LPC, MFT-TL, APSW, or LPC-IT • Experience with Epic • Strong communication skills • Teamwork skills • Crisis intervention skills • No experience required
