📋 Description

• Provide remote care to patients under the direction of a qualified health care provider • Support different clinics and specialty clinics within the scope of the nursing license • Complete virtual and telephonic patient program admissions, assessments, education, care planning, care coordination, and remote patient monitoring escalation follow-up • Deliver safe, quality care according to provider orders, department protocols, and nursing care standards • Document treatment plans, care coordination, regulatory requirements, and reimbursement information • Communicate with management, patient care teams, clinical staff, providers, patients, and caregivers • Use electronic technology for data collection, documentation, information gathering, and communication • Apply evidence-based practice and scientific knowledge to age-specific and clinical nursing needs • Participate in program development and process improvement • Perform other duties as assigned

🎯 Requirements

• Must live in Wisconsin • Must be within driving distance of the Madison metro clinics • Must hold a Wisconsin nursing license or applicable compact-state RN license • Graduate of an accredited school of nursing or education equivalency for licensing • One year of registered nurse experience • Ability to work Monday–Friday, 8 a.m.–5 p.m. during training • Ability to transition to four 10-hour days, 7 a.m.–5 p.m. • Ability to use electronic technology for data collection, documentation, information gathering, and communication • Ability to meet physical requirements including standing, walking, lifting, carrying, pushing, pulling, keyboard use, and occasional driving
