📋 Description

• Perform concurrent analytical reviews of clinical and coding data from point of entry to discharge • Evaluate documentation and assign principal and secondary diagnoses and procedures for accurate DRG, ROM, and SOI assignment • Conduct initial and follow-up patient record reviews and support working or final DRG assignments at discharge • Query physicians regarding missing, unclear, or conflicting health record documentation • Identify diagnostic testing reporting issues proactively • Develop, present, and maintain compliant clinical documentation queries • Educate physicians and healthcare providers on clinical documentation improvement and accurate, complete health record documentation • Attend department meetings to review documentation-related issues • Research clinical topics, coding guidelines, regulatory policies, healthcare trends, and healthcare economics • Collaborate with coding professionals to reconcile DRGs and resolve mismatches using the escalation policy • Troubleshoot documentation and communication problems and escalate appropriately • Clarify clinical issues with coding professionals to support accurate DRG, SOI, and ROM assignments • Assist with mortality review and risk adjustment using third-party models • Impart knowledge to providers and other healthcare team members • Maintain expertise through continuing education programs • Apply evidence-based practice and scientific knowledge to nursing practice • Work in a constant state of alertness and safe manner • Perform other duties as assigned

🎯 Requirements

• Graduate of an accredited school of nursing, PA, NP, or medical school, or Associate's degree and Certified Clinical Documentation Specialist (CCDS) certification from ACDIS • Two years' experience in an acute care setting or relevant experience • Required professional license and/or certification appropriate to the state of work location • CCDS certification from the Association of Clinical Documentation Improvement Specialists (ACDIS), or qualifying medical degree, physician assistant, physician, registered nurse, advanced practice nurse/APRN, or related listed credential • For Illinois work locations: applicable Illinois IDFPR license or certification • For Missouri work locations: applicable Missouri professional registration/license or compact-state RN license • For Oklahoma work locations: applicable Oklahoma board license/certification or compact-state RN license • For Wisconsin work locations: applicable Wisconsin DSPS license/certification or compact-state RN license • Ability to perform frequent lifting/carrying and pushing/pulling of objects weighing 0–25 lbs. • Ability to perform frequent sitting, standing, walking, reaching, repetitive movements, keyboard use, and data entry • Ability to perform occasional lifting/carrying and pushing/pulling of objects weighing 25–50 lbs. • Ability to hear alarms and communicate information orally • Ability to work in a constant state of alertness and safe manner
