📋 Description

• Optimize medication management so physicians can focus more time on patient care and support care coordination for assigned patient cohorts • Work closely with the care coordination team and physicians to ensure patients receive intended therapies, understand medication therapy, and receive adherence coaching • Provide telephonic follow-up for medication reconciliation and medication management, including transitional care and chronic disease support • Complete assigned medication management tasks and appointments according to prescribed workflows • Coordinate transitional management, including obtaining medication orders, completing medication reconciliation, and ensuring medication is provided until PCP follow-up • Assess patient adherence behaviors, eliminate barriers to adherence, and assess therapy appropriateness • Document services in the Engage application, EnterpriseRx, and care management programs • Work with central or local pharmacies to obtain and process prescriptions • Communicate with providers regarding services provided • Support member education goals regarding specific chronic conditions based on the Plan of Care • Support the evolution of pharmacy services delivery for key patient populations, if applicable • Administer tasks detailed in member-specific Plans of Care • Comply with customer service expectations, departmental policies, procedures, and job scope limitations • Participate in departmental and company in-services • Perform other related duties as assigned

🎯 Requirements

• PharmD • Texas state pharmacy license and/or assigned market license • Residential state license in good standing • Clinically required licenses in good standing • PIC-required licenses in good standing • Strong patient engagement skills, including active listening and de-escalation intervention training • Ability to teach complex topics in an understandable manner • Strong organizational and time management skills • Ability to work with little supervision while handling numerous projects simultaneously • Excellent written and verbal communication skills, especially professional and effective telephone communication • Computer literacy in Microsoft Word and Excel
