📋 Description

• Develop, implement, and evaluate population health initiatives centered around medication management, adherence, and optimizing therapeutic outcomes • Utilize health data analytics to identify trends, gaps in care, and opportunities for intervention to improve medication-related outcomes • Collaborate with healthcare providers on medication selection, dosing, and monitoring using population-specific needs and evidence-based guidelines • Design and implement patient medication education programs covering understanding, adherence, and potential side effects • Coordinate care plans and interventions with physicians, nurses, social workers, and other healthcare professionals • Participate in quality improvement projects to enhance medication-related outcomes, reduce adverse events, and promote evidence-based practices • Stay updated with pharmaceutical regulations and ensure compliance with relevant laws, policies, and guidelines • Provide pharmacy team leadership, mentor junior staff, and contribute to departmental goals and initiatives

🎯 Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited institution • Active pharmacist licensure in both your state of residence and Texas, in good standing • Strong analytical and data interpretation skills • Experience in pharmacy practice, preferably in a population health or managed care setting • Excellent communication, collaboration, and leadership abilities • Knowledge of healthcare regulations, policies, and quality improvement methodologies • Spanish speaker preferred • Advanced training or certification in population health management or a related field preferred
