📋 Description

• Develop, implement, and evaluate population health initiatives centered around medication management, adherence, and optimizing therapeutic outcomes. • Utilize health data analytics to identify trends, gaps in care, and opportunities for intervention to improve medication-related outcomes for the target population. • Collaborate with healthcare providers to ensure appropriate medication selection, dosing, and monitoring, considering population-specific needs and evidence-based guidelines. • Design and implement programs to educate patients about their medications, ensuring understanding, adherence, and management of potential side effects. • Work closely with healthcare teams, including physicians, nurses, social workers, and other healthcare professionals, to coordinate care plans and interventions for improved patient outcomes. • Participate in quality improvement projects aimed at enhancing medication-related outcomes, reducing adverse events, and promoting evidence-based practices. • Stay updated with pharmaceutical regulations, ensuring compliance with relevant laws, policies, and guidelines. • Provide leadership within the pharmacy team, mentorship to junior staff, and contribute to departmental goals and initiatives.

🎯 Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited institution. • Active pharmacist licensure in both your state of residence and TX in good standing. • Advanced training or certification in population health management or a related field (preferred). • Strong analytical and data interpretation skills. • Experience in pharmacy practice, preferably in a population health or managed care setting. • Excellent communication, collaboration, and leadership abilities. • Knowledge of healthcare regulations, policies, and quality improvement methodologies. • Spanish speaker (Preferred)
