📋 Description

• Assist in coordinating, implementing, and monitoring pharmaceutical services for a specific population or community • Support medication reconciliation processes • Collaborate with pharmacists and healthcare providers to optimize medication regimens • Review patient profiles for medication-related issues and discrepancies • Collect and analyze medication-related data to identify trends and improvement opportunities • Generate reports on medication adherence, therapeutic outcomes, and relevant metrics • Coordinate pharmaceutical services with healthcare teams • Communicate with patients, caregivers, and healthcare professionals about medication-related concerns • Participate in population health programs promoting medication adherence and preventive care • Support initiatives to improve health outcomes and reduce medication-related disparities • Ensure compliance with regulatory standards, pharmacy laws, and organizational policies • Contribute to pharmacy quality improvement efforts

🎯 Requirements

• High school diploma or equivalent • Completion of a pharmacy technician program preferred • Active licensure as a pharmacy technician in Texas and the state of residence • Strong understanding of pharmacy operations, medication management, and healthcare systems • Excellent communication, interpersonal, and organizational skills • Proficiency using pharmacy software and computer systems for data entry and analysis • Ability to work collaboratively in a multidisciplinary team environment • Fluent Vietnamese speaking ability
