📋 Description

• Conduct virtual assessments of patients with suspected dementia, including baseline cognitive assessments and laboratory testing orders as needed • Conduct medication reviews and recommend reducing high-risk medications or starting symptom-management medications where indicated • Develop comprehensive care plans with care navigators • Coordinate care with care navigators, primary care teams, external providers, and specialists • Support urgent care and other geriatric practice needs as the practice evolves • Participate in multidisciplinary huddles and shadow care navigators and the Head of Medical • Advise care navigators on patients' clinical needs • Help onboard future colleagues and improve and scale clinical programs

🎯 Requirements

• Family or adult-gerontology nurse practitioner with 1 year minimum experience • Licensed in Illinois • Ability to be licensed to practice virtual/outpatient care in any state • Must not be excluded from participating in any state or federal healthcare programs • Passion for caring for patients with dementia • Comfort with learning new technology systems • Comfortable working from home in an all-remote team • Experience with electronic medical record systems, including Athena Health
