📋 Description

• Support Remote Pharmacy Services by collecting and documenting accurate patient medication histories • Assist pharmacists and perform related clinical and administrative tasks • Contact patients and providers to address gaps in care, primarily through phone outreach • Support assigned clients and thoroughly document interactions and outcomes in the call tracking system • Gather patient encounter details, including allergies, medication history, medical conditions, and other relevant information • Contact patients to reconcile medication lists and collect information needed to support care • Coordinate with providers and interdisciplinary teams to support medication management services

🎯 Requirements

• Pharmacy Technician National certification is required • Experience with Medication Therapy Management is preferred • Experience with prescription data entry is preferred • Ability to communicate effectively by email and phone • Excellent interpersonal skills and a professional communication style • Experience with Google G-Suite is preferred • Internet connection through a cable broadband or fiber optic internet service provider • Minimum internet speeds of 10Mbps download/5Mbps upload for home working
