📋 Description

• Contact patients and providers to address gaps in care, primarily through phone outreach • Support an assigned client and thoroughly document interactions and outcomes in the call tracking system • Gather patient encounter details, including allergies, medication history, and medical conditions • Contact patients to reconcile medication lists and collect information needed to support care • Coordinate with providers and interdisciplinary teams to support medication management services • Assist pharmacists with related clinical and administrative tasks

🎯 Requirements

• National Pharmacy Technician certification is required • Experience with Medication Therapy Management is preferred • Experience with prescription data entry is preferred • Ability to communicate effectively by email and phone • Excellent interpersonal skills and a professional communication style • Experience with Google G-Suite is preferred • Availability Monday–Friday, between 9a–8p EST • Home internet through cable broadband or fiber optic service with at least 10 Mbps download and 5 Mbps upload
