📋 Description

• Provide complex care coordination and monitoring to members with primary intellectual or developmental disabilities and possible secondary mental health, substance use, or complex medical needs • Assist with integrated care management through multidisciplinary collaboration and referrals • Assist with the TCM opt-in/opt-out process • Complete care needs screenings, 90-day transition plans, CMCAs, and care plan development or updates • Coordinate services as needed and appropriate • Serve as the primary point of contact for assigned members, providing ongoing care management and advocacy • Conduct comprehensive, person-centered assessments of member needs • Develop, implement, and monitor individualized care plans with members, families, and multidisciplinary support teams • Coordinate services across providers, agencies, and community resources • Update assessments and care plans as member needs and circumstances change • Focus primarily on members diagnosed with Autism Spectrum Disorder, particularly children aged 18 months to 15 years • Work remotely from 8:30 am to 5:00 pm and travel within the community to meet members and attend meetings

🎯 Requirements

• Must meet Qualified Professional (QP) status • Fully licensed by the NC governing board regulating Human Services professions; OR fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing and four years of full-time experience with the MH/DD/SA population served • Master's degree in a human service field and one year of full-time MH/DD/SA experience, or one year of supervised alcoholism and drug abuse counseling experience • Bachelor's degree in a human service field and two years of full-time MH/DD/SA experience, or two years of supervised alcoholism and drug abuse counseling experience • Bachelor's degree in another field and four years of full-time MH/DD/SA experience, or four years of supervised alcoholism and drug abuse counseling experience • If serving LTSS members, at least two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience • Must have a valid driver's license • Must reside within Trillium's South Central or Mid State region: Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, Wayne, Anson, Guilford, Montgomery, Randolph, or Richmond counties • Must be able to travel within catchment as required • Must pass a drug test as a condition of employment • Resume must include employer names, dates of service, average weekly hours, essential duties, education details, and licensure/certification information
