📋 Description

• Provide clinical assessment and oversight to support safe transitions between care settings • Evaluate medical needs and determine appropriate levels of care during transitions • Collaborate with providers and care teams to ensure clinical alignment of services and treatment plans • Monitor clinical status during transitions and identify changes requiring intervention • Support coordination of medical services and facilitate referrals to appropriate healthcare providers

🎯 Requirements

• Associate's degree and one (1) year of experience as a Registered Nurse (RN) • Fully licensed RN in the state of North Carolina • Must have a valid driver’s license • Must reside within Trillium’s Southern Region: Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland counties • Must be able to travel within catchment as required • Must be legally authorized to work in the United States without requiring sponsorship for employment now or in the future • All candidates must pass a drug test as a condition of employment • Preferred: Experience supporting individuals in the community such as in adult care homes, community hospital, or home
