📋 Description

• Provide clinical care management and population health support for assigned members • Focus on prevention, wellness, and longitudinal health outcomes • Apply nursing judgment as a licensed registered nurse to analyze data and identify care needs • Coordinate services using population health management approaches that improve outcomes and reduce reliance on episodic care • Conduct clinical assessments to identify member health needs, risks, and care gaps • Develop and implement care plans for preventive care, chronic condition management, and wellness goals • Coordinate services across providers, care teams, and community resources • Monitor member progress and adjust care plans based on clinical indicators and outcomes • Provide health education and coaching to support self-management and engagement

🎯 Requirements

• Associate’s degree in Nursing and one (1) year of experience as a Registered Nurse • Active North Carolina Licensure as a Registered Nurse (RN) • Valid driver’s license • Must reside within North Carolina • Must be able to travel within catchment as required • All candidates must pass a drug test as a condition of employment
