• Provide care management and transition coordination for members in the Trillium Support Transition and Reentry (STAR) Program • Ensure members are linked to medically necessary services and community supports through comprehensive transition planning • Develop and implement discharge and transition plans, including supported housing • Document member progress, referrals, and engagement activities • Maintain documentation in Trillium’s Care Management systems and TCL database according to policy
• Must meet Qualified Professional (QP) status with at least one (1) year experience with the forensic population • Fully licensed by the NC governing board regulating Human Services professions OR Fully licensed RN with four (4) years of experience with the MH/DD/SA population • A graduate with a Master’s degree in human service field and one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience OR a bachelor’s degree in human service field with two years of experience OR a bachelor’s degree in non-human services with four years of experience • Must have a valid driver’s license • Must reside within North Carolina within 75 miles of Duplin County • Must be able to travel within catchment as required
