📋 Description

• Research, collect and analyze information to identify opportunities, develop solutions and lead through resolution • Collaborate on performance improvement activities based on program efficiency and patient experience outcomes • Distribute analytical reports • Use multiple system applications to perform analysis, create reports and develop educational materials • Research and compile information for ad-hoc operational projects and initiatives • Synthesize and analyze data, provide detailed summaries and graphical presentations illustrating trends, and recommend practical options or solutions • Leverage program and operational data and measurements to define and demonstrate progress, ROI and impacts • Obtain appropriate clinical documentation through extensive interaction with providers and coding staff • Validate accurate assignment of working HCC diagnoses affecting RAF scores in the ambulatory patient population • Conduct prospective and retrospective quality reviews of ambulatory patient records, document findings and track key information • Identify documentation clarification needs and engage physicians, advanced practice providers and healthcare professionals to resolve discrepancies • Ensure medical record documentation is accurate, complete and compliant, supporting conditions and medical necessity • Apply knowledge of payment structures, outpatient reimbursement models, provider documentation and HCC risk adjustment • Identify patterns and trends impacting documentation and coding and act as a technical resource for documentation, coding and billing regulations

🎯 Requirements

• Associate’s degree in Business, healthcare, Nursing or related field, or equivalent combination of education & experience • Experience with compliant healthcare documentation, HCC coding requirements, alternate payment models in a multi-facility, integrated health care delivery system, revenue cycle or consulting experience • CRC, CDI, CCDS-O, CDEO, or CDIP credential with coding or clinical documentation integrity experience • Knowledge of applicable federal, state and local laws/regulations, Trinity Health Integrity & Compliance Program, Code of Conduct, policies, procedures and guidelines • Ability to use multiple system applications for analysis, reporting and educational materials • Ability to analyze data, create reports and develop educational materials • Ability to read small print, hear sounds and verbal communications, use computers and technology, and perform required physical activities • Ability to lift a maximum of 30 pounds unassisted occasionally • Ability to maintain a safe working environment and use PPE • Bachelor’s degree in nursing, HIM or related healthcare field is nice to have
