📋 Description

• Facilitates communication and coordinates care with the patient, primary and specialty care physician, and hospital departments on standardized care pathways • Ensures availability, access, and timeliness of services to optimize quality of care, coordination, and patient experience • Provides evaluation, education, coordination, and referral to patients identified with an incidental radiological finding with recommended follow-up • Works collaboratively with other FIND/LCS staff

🎯 Requirements

• Graduate of an accredited school of Nursing and Bachelor of Science Degree in Nursing • Three (3) to five (5) years of progressive clinical nursing experience is required • Current license to practice as a Registered Nurse in the State of Michigan • Problem solving skills and ability to initiate creative solutions to problems • Demonstrated ability to expertly navigate a variety of software applications • Working knowledge of Radiology procedures protocols and processes, preferred • Effective organizational skills • Must possess a personal presence characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health
