📋 Description

• Provide extensive nutritional and lifestyle education to patients via telehealth to enhance their health and reduce healthcare costs. • Work closely with patients to achieve and maintain healthy weight loss. • Guide patients to appropriate community resources to support their health journey. • Assess nutritional and lifestyle outcomes, developing impactful reports to track progress. • Formulate and communicate clinical recommendations and visit summaries to healthcare providers and patients. • Proactively identify and address barriers to effective patient care. • Partner with departments to leverage technology and health information for better patient outcomes. • Deliver clinical reports and presentations in partnership with the account manager, educating clients on their benefit plans. • Educate clients on clinical programs and solutions to increase adoption and implementation. • Plan, organize, and direct responsibilities as delegated by the Clinical Innovation Pharmacist.

🎯 Requirements

• Bachelor’s degree required • Registered Dietitian Certification (CDR) • State Certification or Licensure • Strong work experience counseling individuals in an outpatient setting with complex disease states to prevent complications • Familiarity with current treatment standards and options for common metabolic conditions, including diabetes and obesity • Proficiency in MS Word, Excel, and Outlook, with experience creating and updating Excel spreadsheets • Ability to work independently with minimal supervision • Skilled in identifying patient diet/lifestyle barriers and developing strategies to overcome them • Significant experience in patient education and engagement • Willing to Travel
