📋 Description
• Perform in-home assessments, clinical monitoring, caregiver training and oversight, and skills-based instruction depending on the client and program assignment • Work across three programs — skilled home health, In-Home Support Services (IHSS), and Independent Living Skills Training (ILST) • Provide clinical autonomy in the field without fixed shift commitments, mandatory overtime, or on-call requirements
🎯 Requirements
• Active, unrestricted Colorado RN license (or compact multistate privilege) • Minimum 1 year clinical nursing experience • Current BLS/CPR certification • Reliable transportation, valid CO driver's license, and current auto insurance • Ability to travel across the Greeley and/or Northglenn service areas • Ability to pass background screening and required health clearances • Home health, hospice, case management, or community health experience (strongly preferred) • Experience with brain injury, neuro rehab, developmental disability, or geriatric populations (strongly preferred) • Comfort with electronic documentation (strongly preferred) • Bilingual English/Spanish (strongly preferred)
