📋 Description

• Provide treatment, management, and guidance to members seeking diabetic reversal and overall health improvement across multiple conditions • Support members in multiple states through remote care management • Engage with health coaches, physicians, chief medical officer, and other healthcare professionals • Conduct health assessments, including laboratory results, medical history, and psychosocial history • Assess symptoms and treat as appropriate, collaborating with members' primary care providers • Deliver care using telemedicine and document in Twin’s clinical platform • Manage a high-risk population and address social determinants of health and population-specific needs • Learn and apply the Twin Model of Care for diabetes and chronic condition reversal • Collaborate with the medical team to provide excellent customer service and member experience • Provide patient education to promote disease prevention and good health • Review medical histories, symptoms, allergies, and medications • Ask situation-specific questions to assess symptoms • Prescribe medications and provide dosage and administration instructions per Twin Health policy • Maintain accurate patient records, including contact details, medical history, medications, allergies, diagnoses, and progress • Work Monday-Friday: three shifts 11am-8pm and two shifts 9am-6pm PST • Perform additional duties as assigned

🎯 Requirements

• ARPN CA state license, preferably licensed in multiple states • Fluent in English and Spanish • 3-5 years of experience as an advanced practice provider • Clinical experience working with multiple populations, including underserved groups • Experience managing patients remotely across different geographic areas and states • 5 or more licenses, with willingness to obtain more • Board-certified in family or internal medicine • Proven experience working as an advanced practice provider • Sound medical knowledge • Ability to consult with patients through virtual communication channels • Experience using technology and data to guide care decisions • Excellent analytical and problem-solving skills • Exceptional communication skills • Patient and compassionate disposition • Detail-oriented • Interest in working with underserved groups
