📋 Description

• Improve the overall quality and completeness of clinical documentation so medical records accurately reflect patients’ severity of illness, clinical needs, and resource utilization • Review death medical records to ensure severity of illness and risk of mortality are captured at the highest level • Review all hospital admission types to ensure severity is captured and educate providers on admission types • Read and analyze health record documentation to identify significant diagnoses, treatments, and procedures affecting acuity and consumed resources • Apply disease-process, surgical-procedure, UHDDS, CMS/NCHS ICD-10 coding-guideline, and coding-classification knowledge • Work with physicians, nursing staff, caregivers, coding staff, and other healthcare team members to clarify clinical documentation • Facilitate accurate documentation, coding, and reimbursement practices and ensure documentation reflects the highest specificity • Develop and facilitate education on clinical documentation improvement, coding guidelines, and regulatory requirements • Perform other duties as assigned

🎯 Requirements

• Registered Nurse (RN) with current license plus 4 years of inpatient clinical experience in Emergency, Critical Care, or Medical/Surgical areas, with experience in Quality, Case Management, Clinical Audit, or Data Abstraction • OR coding professional with CCS certification and 6 years of inpatient coding experience • Coding professionals must have documented education in anatomy/physiology and pharmacology • CCDS required within 6 months of hire • Current CCDS is preferred • Resume required • List of three professional references required, including name, email, and business title • Proof of legal authority to work in the United States on the first day of employment • Subject to pre-employment screening, including criminal background, drug testing, and/or education verification • Ability to apply ICD-10 Official Coding Guidelines, UHDDS definitions, disease-process and surgical-procedure knowledge • Ability to use the Vizient calculator • Ability to analyze health record documentation and identify diagnoses, treatments, and procedures affecting acuity and resource utilization
